Because insurance benefits vary in coverage, we strongly suggest that you contact your insurance carrier for specific health coverage.
New patients may fill out a
New Patient Form
prior to your appointment.
Sick Visits
If your child is sick, we do offer same day appointments as available. Though we do allow walk in visits, we ask that you be patient as we will see appointment made patients prior to walk in patients. We are able to perform strep tests, urine analysis, flu tests, and pregnancy tests in office. Send out tests are available on a case by case basis. Blood work is typically performed at an outside facility, with orders written by the provider.
Sports Physical
Most school and after school sports programs require a sports physical form to be completed by a provider. We are happy to complete these exams in our office for any age! Our providers perform a thorough physical examination, review past medical history and family medical history to ensure your child is safe for physical activity. This appointment can be made during a well child appointment or separately as needed and the signed form lasts for up to 1 year.

Nurse Only
Some appointments do not require a provider to see your child. These appointments typically occur after seeing a provider and are "follow up" on a particular concern. For example, you may see a nurse only for a repeat hearing or vision test. If our office is extremely busy and you are unable to see a provider, a nurse may be able to perform simple tests such as a flu or strep test. These appointments are made on a case by case basis.
Such visits are reviewed by a provider and if there are any concerns in office, a provider may be able to briefly see your child.
